What this policy covers
It covers data processed when you visit the dahamed.sk website.
Medical records and patient data are processed at the surgery under Act
No. 576/2004 Coll. on healthcare and Act No. 18/2018 Coll.
on personal data protection. We inform you about that in person when you
register. The website is not a place where health data is collected.
Please do not send us details about your health by e‑mail. Ordinary
e‑mail is not a secure channel. Raise medical questions by phone or in
person at the surgery.

What data the website processes
The site has no contact form, creates no user accounts and
does not profile visitors. Only the following is processed:

Server logs. IP address, date and time of access, the
requested address, browser and operating system type. Purpose: operation,
security and fault diagnosis. Legal basis: legitimate interest,
Art. 6(1)(f) GDPR.

Cookie consent record. Your choice in the consent bar and
the time it was given, stored in your browser as
cc_cookie.
Purpose: demonstrating consent. Legal basis: legal obligation and your consent.

Correspondence. If you write or call, we process what is
needed to answer you. Legal basis: legitimate interest, or steps taken
before entering into a relationship.

Site usage (Google Analytics 4). We measure which
pages are viewed, for how long and where visitors arrive from, so we can
improve the content. Purpose: audience statistics. Legal basis:
your consent, Art. 6(1)(a) GDPR. Without consent
no analytics cookies or identifiers are stored.
Necessary cookies are needed to run the site and to store your
choice in the consent bar. They are always set.
Analytics cookies from Google Analytics 4
(_ga, _ga_*) are set
only after you consent. Until then the site runs in Google
Consent Mode with storage denied, meaning no analytics cookies and no device
identifiers.
Even when you decline, Google Analytics sends an anonymous signal with no
cookies and no device identifiers. It is used only to estimate overall
traffic and cannot be used to identify you.
We use no marketing tools. The consent bar offers that
category for future use. If we introduce it, it will run only after your
consent and we will update this policy.
You can change or withdraw your choice at any time using the
Cookie settings button in the bottom left corner of the page.
The consent record is valid for 365 days.

Recipients and transfers
Loading the page requests content from the following providers, which exposes
your IP address to them:
- Google Analytics 4 (googletagmanager.com, google-analytics.com): audience statistics, only with your consent. Google Ireland Limited, Google LLC.
- Google Fonts (fonts.googleapis.com, fonts.gstatic.com): typefaces. Google Ireland Limited, Google LLC.
- jsDelivr (cdn.jsdelivr.net): the consent bar library.
- unpkg (unpkg.com): the map library. Loaded only once you approach the map.
- CARTO (basemaps.cartocdn.com): map tiles. Loaded only once you approach the map.
- WebSupport s.r.o.: site hosting and server logs.
Some of these providers may process data outside the European Economic Area,
in particular in the USA. Such transfers rely on standard contractual clauses
or on an adequacy decision.
We do not sell data and we do not use it for advertising profiling.
- Server logs: for the period set by the hosting provider.
- Cookie consent record: 365 days, or until withdrawn.
- Data in Google Analytics 4: per the retention period configured in the service, at most 14 months for event data.
- E‑mail and phone correspondence: as long as needed to deal with it and to meet legal obligations.
In relation to your personal data you have the right:
- of access to the data and to a copy of it,
- to rectification of inaccurate or incomplete data,
- to erasure,
- to restriction of processing,
- to data portability,
- to object to processing based on legitimate interest,
- to withdraw consent at any time, without affecting the lawfulness of processing before withdrawal.
